Informed Consent
Please read this information carefully before proceeding.
1. Map My Gene LLC (hereinafter referred to as “MMG”) has undertaken to arrange for the completion of the application form and the collection of the buccal sample(s) required to facilitate the above-mentioned test(s).
2. MMG agrees to maintain the confidentiality of the customer’s information and not to disclose test results or reports without the customer’s consent. This includes maintaining the confidentiality of test sample(s) and test report(s).
Important Information
1. The buccal sample(s) provided for testing must belong to the person being tested.
2. The test sample(s) will be submitted solely for the purpose of conducting the selected genetic test(s), and the resulting test report(s) will be issued by the laboratory.
3. The test(s) do not constitute a medical or clinical diagnosis and are not intended to be a medical service. Genetic factors are only one part of a person’s health, development, talents and traits. Environmental factors, lifestyle, age and other variables may also influence these characteristics and health outcomes. A genetic predisposition to certain diseases, talents or traits does not mean that a person will inevitably develop the disease or exhibit the associated talents or traits. Conversely, the absence of a reported genetic predisposition does not guarantee that a person will not develop the disease or exhibit the associated talents or traits.
4. Test results are personal and confidential. Customers are responsible for any action they may or may not take based on their results. MMG shall not be held responsible for the manner in which customers use or disclose their results.
5. All tests sold are non-refundable.
6. If a test sample is lost, misplaced, damaged, unusable, insufficient or corrupted, a re-test may be required. Where a re-test is required for these reasons, it will be conducted at no additional charge.
Consent
By proceeding with the purchase, customers will be asked to confirm that they have read and understood this Informed Consent and voluntarily consent to the collection, processing and genetic testing of their DNA sample for the selected genetic test(s).
